There are three ways to update your information:

  • Click here to download a PDF version of the membership application form which you can complete and fax to 905-513-1248. Please note:  The form is on legal-size paper, so please allow for this when printing it out!

  • Call SCL's National Office at 905-513-7300 or 1-866-456-1231 and we will be happy to either take your information by phone.

  • Cliquez ici pour télécharger une version PDF de notre formulaire d’adhésion que vous pourrez compléter et nous télécopier au (905) 513-1248. N.B. : Le formulaire étant dans le format U.S. Legal, or vous devrez en tenir compte lors de l’impression !
  • Contactez le bureau national de CAL au (905) 513-7300 or au 1 (866) 456-1231 et nous serons heureux de recueillir vos renseignements par téléphone ou vous envoyer une version imprimée du formulaire d’adhésion.
